Case Management
A client-centered care coordination service model designed
to help People Living with HIV/AIDS (PLWHA) obtain
social support, enhance their health outcomes and improve
independent lifestyle skills. Core elements include an assessment
of client’s needs, the development of a comprehensive
service plan, referrals and linkage to appropriate resources,
and coordination of service delivery.
Transportation
Program
The transportation program is designed to enable HIV positive
persons to access medical, mental and support services to
optimize health outcomes. Demand for transportation is met
using four sources: bus cards/tokens, van services, taxi and
wheelchair services. Staff works with case managers to
address the client’s specific transportation needs and develop
the best service plan for each client.
Linguistic Service Component
At LCS we believe that clients not only have the right to prompt and adequate medical service but that they also have the right to communicate in the language they feel most comfortable communicating in.
LCS Linguistic Services is a unique component of our care model that empowers monolingual Spanish speaking persons living with HIV/AIDS to effectively manage their health through LCS interpreters.
The mission of this component is to assist in the provider-patient relationship and improve the health outcomes of patients with limited English proficiency. LCS interpreters can provide translation services in person, phone based translation for Spanish speaking clients.
